The Aflac Group Hospital Indemnity plan benefits include the following:

• Hospital Admission Benefit – $500
• Hospital Confinement Benefit – $100
• Hospital Intensive Care Benefit – $100
• Intermediate Intensive Care Step-Down Unit – $50

Please note the Hospital Intensive Care Benefit and the Intermediate Intensive Care Step-Down Unit Benefits are payable in addition to the Hospital Confinement Benefit.

Sample Per Paycheck Rates:

Mammography tests performed while an insured’s coverage is in force are eligible for a $100 benefit once per calendar year based on the insured’s age.
